It might be difficult to adjust to a new stage of life. That’s why Wellness Care is here to help participants navigate life’s ups and downs. We can assist with a variety of life-stage transitions, including starting or changing schools, moving home or into supported housing, and even getting out and about more in the community.
We assist people in making plans for change, including establishing coping techniques for the stress that change might bring. Our approach is tailored to everyone, taking into consideration their strengths and needs. All of this is designed to assist participants in achieving their short and long-term objectives.
Preparation of schedules (daily, weekly, and/or longer-term) is one of the services we offer. This is to ensure that the assistance offered is beneficial to the participant’s life stage transition goals. We can also assist with the budgeting process, ensuring that expenses are anticipated and paid on time.
We offer both short- and long-term help for people transitioning through life changes. The goal is to improve the participant’s ability to coordinate their supports, as well as to help them live comfortably and engage in their communities.
