community nursing care program
Participants require varying types of medical care and attention depending on their disability. Wellness Care’s community nursing care program emphasizes the need for participants to become self-sufficient in terms of their health as a key component of the program. Participants in the community nursing care program are taught how to look after their health in situations such as medicine, wound care, hygiene, and assistance with showering or dressing. A registered nurse or medical practitioner is usually assigned to monitor the patient’s development and adjust the treatment and medication as needed. Some of the services provided by the community nursing care program are as follows:
- Assessment and a visiting nurse
- Creating treatment strategies
- Management of the case
- Organizing your medication
- Administration and management of medications
- Wound treatment
- Stoma treatment
- Support for the lungs
- Continence management and advice
- Palliative care assistance
- Management of diabetes
- Alzheimer’s disease management
- After-hospital-stay care
- Nursing care for the night
- Vital signs are being monitored.
- Participants and their families will receive an education.
We have certified and experienced medical experts on staff at Wellness Care that can provide community nursing care under the NDIS scheme. We offer a no-obligation consultation in the comfort of your own home to listen to and understand your requirements.
Each participant who gets community nursing care as part of their NDIS support plan is assigned to a medical staff person who will keep in touch with them to discuss, monitor, and provide feedback on their medication, treatments, and progress. Aside from that, our medical staff members provide guidance on how to engage in basic self-care and nursing so that one may perform simple tasks such as wound care and first aid on their own.
